Lartey Wellness Group offers professional documentation services to support your mental health treatment and related needs. These may include letters for school, work, disability, and emotional support animals (ESAs). Please note that requests are subject to clinical approval and must meet specific requirements.

Fee
Flat Rate: $100 per letter or form request
This fee is not billable to insurance.

Important Requirements
1.These services are not billable to insurance.
2.A credit card must be saved on file
3.You must be an active client currently participating in treatment with a therapist or nurse practitioner at Lartey Wellness Group.
4.We do not guarantee that your request will be fulfilled during or immediately after your intake session.
5.Only your treating provider can determine whether the requested documentation is clinically appropriate and justified.
6.Most requests require at least three therapy or psychiatry sessions before documentation can be issued.

Letter of Attendance & Letter Confirming Diagnosis are free upon request
