How to Submit a Grievance/Complaint

It is the policy of Lartey Wellness Group to encourage all of our clients to state complaints and/or grievances if they believe their rights have been violated, and to pursue a resolution to their concerns in a structured format that provides fair and equitable results through due process. 

If you feel that your rights have been violated in any way, or would like to submit a complaint, please do so using the following method:

  1. Log on to the Accreditation Now online platform by using this link: https://www.accreditationnow.com/client-grievance-form.asp

  2. Login using the credentials below:
    Login: 10061313 
    Password: lwgclient3

  3. Fill out as much of the information as possible and then submit the form.
    Mandatory fields include Date, Time, Location (if applicable), Name, Phone and Email. The Address fields are optional.

  4. Describe your complaint in as much detail as possible.

  5. Describe why you believe the action was wrongful, illegal, or unlawful.

  6. Describe the resolution, or desired outcome, you are seeking.

  7. To add witnesses, type in the name and click on the + symbol.

  8. Enter a password that you will use to retrieve a response from your grievance officer. Then select submit.

  9. You will see a box that contains your company number and report number. You will need this information, along with the password you created, to retrieve a response from the grievance officer.

  10. Grievances will be responded to within 5 days of receipt. To retrieve a response, go to AccreditationNow.com, click on the “Client tab” then “Client Grievance Report Status.”

  11. Enter the Company Number: 10061313, the Report Number (provided when you submitted the report), and the Password you created when you submitted the report. Click the Login button.
